Research opt-out form

Please use this form to let us know that you choose to opt out of the classroom educational research/data collection.

If you have consented to having your class recorded with audio or video capture, you may use this form to change your mind.

This form will allow you to change your mind in one or more of the following ways:

• you can tell us to delete recordings of a class or classes that were made previously;
• you can tell us that you don't want us to record "real time" for a class that is just starting;
• you can tell us that you've changed your mind about being recorded in future sessions of this class.

NOTE: The latest that you can tell us to delete previous recordings is one day after the last day of classes for the current semester.
