Passport for a U.S.-Born Infant

U.S. citizenship is available to babies born in the United States. In order to obtain a passport for your U.S.-born infant, please take the following steps:

  1. Obtain an official birth certificate from the Bureau of Vital Statistics. This certificate may have already been sent to you, but if not, you will need to check with the hospital where the baby was born. The information may be available from the hospital itself. You can also call the Bureau of Vital Statistics at 412-565-5113.
  2. Apply for a social security number for the baby. Often, this process happens automatically for babies born in a hospital in the U.S. You may also view information regarding applying for an SSN and directions to local Social Security Office locations. 
  3. Apply for the passport. A listing of locations where you may apply for a passport is included below. To apply, you must have available your child's social security number, certified birth certificate, your own social security card (if you have one), another form of identification for yourself (i.e., passport), two "passport" photographs of the child, and check or money orders for the fees ($110 application fee; $25 processing or execution fee). The child must appear in person when the application is made. Both parents must be present or the parent applying for the passport must have a signed, notarized Form DS-3053 (Statement of Consent: Issuance of a Passport to a Minor Under Age 14) from the parent who is absent.

In-Person Passport Applications

In Pittsburgh, passport applications are accepted at the following locations:

  • Allegheny County Courthouse, 414 Grant Street, First Floor Pittsburgh, PA 15219, 412-350-4180
    • Only accepts personal check or money order for fees.  ($110 payable to US Department of State; $25 payable to Allegheny County Treasurer)
  • Local Post Offices
    • Oakland Post Office, 347 S. Bouquet Street, 412-621-9708
    • Squirrel Hill Post Office, 1800 Murray Avenue, 800-275-8777
    • The US Post Office only accepts personal check or money order for the $110 application fee (payable to US Department of State). They will accept cash, money order, check, and debit/credit card for the $25 execution fee. Some US Post Offices are able to provide passport photographs as well.
