Pre-Completion OPT Applications Materials

Below is a list of all of the application materials you will need to apply for Pre-Completion OPT. You will need to submit these materials in this order, so be sure to carefully review this list before submitting your application.

  1. Fee information can be found on the U.S. Citizenship and Immigration Services (USCIS) website. You can pay with:
    1. A U.S. check or money order – you’ll need to make it payable to the “U.S. Department of Homeland Security.” No cash!
    2. A credit card – to pay by credit card, you will need to fill out Form G-1450 and attach it to your application.
  2. Form G-1145. This form allows you to receive email or text confirmation (within one week) of your receipt number from the Lockbox facility. Clip the form to the front of the OPT application.
  3. Two “passport” photos. Print your name and write your admission number (I-94#) on the back of each photo. When preparing for your “passport” photos remember:
    1. No glasses can be worn in the “passport” photos. Use powder to prevent glaring, and be sure there are no shadows in your photos!
    2. You may put the pictures and check in an envelope and attach them carefully to the front of the I-765. Be sure not to staple the photos!
    3. You can have your “passport” photos taken at:

  4. I-765 Form (follow the instructions for filling out the form).
  5. Sign the new Pre-Completion OPT I-20 at the bottom of page one.
  6. A copy (not the original) of pages one and two of the new Pre-Completion OPT I-20 from OIE with the request for OPT is listed on page two.
  7. I-20 cannot be more than 30 days old when it arrives at USCIS, or OPT will be denied.
  8. A copy of all previously issued I-20 documents from your current degree level (signed!). If missing I-20(s), please write a cover letter.
  9. A copy of your I-94 Record.
    1. Printout of the electronic I-94 Record – do not send a copy of your "Travel History Page."
  10. A copy of the photo page (identification page) and expiration date page of your passport.
  11. A copy of your passport's most recent F-1 visa sticker (even if expired).
  12. If applicable, a copy of any previous Employment Authorization Document (EAD) card(s).
