Remember to Complete Your Daily Self-Assessment Survey


The Daily Self-Assessment for students is part of "A Tartan's Responsibility."

To help mitigate the spread of COVID-19 on campus and to help support the health and well-being of the CMU community and its surrounding neighborhoods, all students, faculty and staff must complete a quick Daily Self-Assessment before beginning their day on campus. Even if you are not coming to campus, you are strongly encouraged to complete the survey.

The Daily Self-Assessment survey includes several yes or no questions. Immediately after submitting your responses, you will be directed to an acknowledgement page that provides guidance and confirms your ability to participate in classes or engage in on-campus activity. Students who answer “yes” to any of the questions are responsible for contacting University Health Services to seek medical guidance. Faculty and staff who answer “yes” to any of the questions are responsible for contacting their health care provider.

Daily reminders to complete the survey will be sent via email and initially by text messages as well.

“All of us play an extraordinarily important role in ensuring that our community stays healthy as we return to campus,” said Daryl Weinert, CMU’s chief of staff, vice president for strategic initiatives and COVID coordinator. “We each must do our part to fulfill our shared responsibilities to take care of ourselves, our fellow Tartans, and our neighbors.
