Conflict of Interest Survey

The university Purchasing Policy mandates that all university buyers who procure goods on behalf of the university via a purchasing card and/or a PO are required to disclose real, perceived or potential purchasing conflicts of interest. If there is no conflict of interest, real, perceived, or potential to report, buyers are required to certify this information, as well. Furthermore, university buyers are prohibited from providing or attempting to provide or offering to provide any kickback; soliciting, accepting or attempting to accept any kickback; or including, directly or indirectly, the amount of any kickback in the contract price.

To comply with the Purchasing Policy, please complete the Conflict of Interest Disclosure and Anti-Kickback Agreement Form by March 10, 2025.

In addition, buyers should be aware and knowledgeable about the contents of the university Purchasing Policy. As a person responsible for the acquisition of goods and services on behalf of the university, you will be asked to verify your understanding of the requirements within the Policy as part of the Conflict-of-Interest Disclosure and Anti-Kickback Agreement Form.
