Missing Financial Aid Documentation

Financial aid eligibility cannot be finalized if required documentation is missing. If you've been identified as one of these students, you will receive a Financial Aid Alert email from The HUB notifying you that documents are missing from your financial aid application. Instructions for submitting missing documents may be found in the chart below.

CSS Profile

 

If applying for institutional financial aid programs, complete the CSS Profile online at cssprofile.org. If you do not have a College Board online account, you will need to create one in order to complete your CSS Profile application. Carnegie Mellon receives your Profile data electronically from CSS. Carnegie Mellon's CSS College Code is 2074. 

FAFSA

Complete your FAFSA (Free Application for Federal Student Aid) at https://fafsa.ed.gov. There are two ways to complete the Free Application for Federal Student Aid (FAFSA) form: redesigned https://fafsa.ed.gov website or a mobile app (available through Google Play or the Apple App Store).

A Department of Education Federal Student Aid (FSA) ID is required. View FSA ID instructions.

Federal Master Promissory Note (MPN) and/or Entrance Counseling Complete online at https://studentloans.govIf you do not wish to borrow Federal Direct Loans, please complete a Loan Adjustment (pdf) form and submit it to The HUB.

Identity and Statement of Educational Purpose

Submit a valid government issued photo ID and the Statement of Educational Purpose in person to The HUB. If you are unable to appear in person, complete the Identity and Statement of Educational Purpose form (pdf) and submit it to The HUB.

Parent CSS Profile

All parents complete a CSS Profile application at http://cssprofile.org. in the case of divorced or separated parents, each parent will complete their own CSS Profile application with different College Board accounts/login credentials. Carnegie Mellon receives your Profile electronically from CSS.

Non-Tax Filer Statement

Complete a Non-Tax Filer form and submit to The HUB:

Parent PLUS Application

Apply online at https://studentloans.gov by selecting 'Parent Borrowers' and 'Apply for a PLUS Loan'.

Parent PLUS Master Promissory Note (MPN) and/or Entrance Counseling

Complete MPN online at https://studentloans.gov by selecting 'Parent Borrowers' and either 'Complete PLUS Loan Master Promissory Note (MPN)' or 'Complete PLUS Counseling'.

Parent/Student Form 1099 Complete (www.irs.gov/forms-instructions) and submit a copy to The HUB. 

Parent/Student Tax Returns and/or W-2s

 

Submit these to IDOCS in one of the following ways:

  1. Upload electronic versions at https://idoc.collegeboard.org/idoc/ (preferred method); or
  2. U.S. Postal Service to: College Board Processing Center, P.O. Box 8570, Portsmouth, NH 03802

Please note that handwritten signatures and dates are required on page 2 of each Form 1040. You may log into your College Board CSS Profile account and select “Check Status” at the top of the page to verify that CSS/IDOC received your documents. If your submitted documents are not reflected under your account, please contact CSS/IDOC.

Parent/Student Tax Return Transcript

This is required for dependent students if selected for federal verification. We recommend requesting a transcript online (view detailed instructions).

CMU Perkins Replacement Loan Packet

Please visit https://fao.andrew.cmu.edu/forms.html, select the CMU Perkins Replacement Loan Application Packet, and follow the on-screen instructions.

Reaffirmation Letter

If you exceed your aggregate or annual federal loan borrowing limits (find limits at studentaid.ed.gov), you may no longer be eligible for any form of federal financial aid until you can reestablish your eligibility. To reestablish your eligibility, you can either repay the excess loan amount in full, or make repayment arrangements with your Federal Loan Servicer (a process called Reaffirmation). In either case, you must provide Student Financial Aid with a letter from your Federal Loan Servicer confirming that you have made arrangements to reestablish your eligibility. Submit this letter to thehub@andrew.cmu.edu

Schedules A, B, C, D, E, K-1

Complete (www.irs.gov/forms-instructions) and submit a copy to The HUB.

Selective Service Registration

Register online at www.sss.gov and submit the confirmation page to The HUB.

Unsubsidized or Parent PLUS Loan Only Form

Print, complete and submit the application (pdf) to The HUB.

U.S. Citizenship Documentation Submit an original copy of U.S. Citizenship Documentation (i.e., U.S. Passport, Certification of Naturalization) in person to the HUB. If you are unable to appear in person, complete the 2018-19 U.S. Citizen/Eligible Non-Citizen Authentication of Documentation form (pdf) and submit it to The HUB.
Verification of Household Information Based on a review of your financial aid application materials, you are required to verify your household information, which also includes information about any household members enrolled in a degree-seeking or certificate program at a college or university.  To complete the form, please visit https://fao.andrew.cmu.edu/forms.html and select the Verification of Household Information.
Yeknik Student Loan Packet Please visit https://fao.andrew.cmu.edu/forms.html, select the Yeknik Student Loan Application Packet, and follow the on-screen instructions.

Submit Initial CSS/IDOC Packet

If you have not already submitted your initial packet of tax documents to CSS/IDOC for the 2019-20 academic year, then you should submit the following along with the CSS/IDOC cover sheet provided to you via email after your CSS Profile completion:

  • Parent 2017 tax return
  • Parent 2017 W-2 form(s)
  • Student 2017 tax return (if required to file)
  • Student 2017 W-2 form(s) (if applicable)
  • Schedule C and K-1 from Forms 1065 and/or 2555 (if applicable)

Documents may be submitted to CSS/IDOC in one of the following ways:
