Family Care Registration

The registration processes below are required to leverage the new child care options being implemented this fall due to the ongoing impact of COVID-19. CMU faculty, staff and graduate students are eligible for these new, temporary child care options pending successful completion of the registration steps and related forms.

If an undergraduate student needs assistance, please contact the concierge at hrfamilycare@andrew.cmu.edu.

Refer to the sections below for specific guidance on registration for each option. You will receive confirmation of your successful registration via email along with a link to request specific day(s) and time(s). 

Please note that registration is now open for all services.

  1. Complete the forms listed:
    1. Child Care Registration Form [pdf] (will also serve as the Emergency Contact Form)
    2. Enrollment Agreement [pdf] (outlines costs and related fees and policies) 
    3. COVID-19 Disclosure [pdf] (acknowledges agreement with detailed safety protocols)
    4. Medical Form [pdf]
  2. Scan completed and signed forms and email to hrfamilycare@andrew.cmu.edu.
  3. Completed forms will be reviewed and processed.
  4. You will receive confirmation of successful registration via email along with a link to schedule your desired day(s) and time(s).
