CMU Daily Self-Assessment

Dear Faculty and Staff:

As part of our shared responsibility to help prevent the spread of the COVID-19 virus on campus, every member of our community must complete a quick Daily Self-Assessment survey each day they plan to be on campus. Those who will not be on campus are also strongly encouraged to complete the survey.

For faculty and staff, the survey is administered online, and includes just four questions that prompt you to consider your health.Immediately after submitting responses, you will be directed to a webpage that will either confirm that you are permitted to come to campus, or provide additional guidance. As we launch the Daily Self-Assessment, you will receive a text message and email reminder at 7 a.m. (Monday through Friday) and 10 a.m. (Saturday and Sunday) to complete the survey. Beginning Monday, August 17, you will only receive reminders via email.

Students are administered a similar Daily Self-Assessment survey. The details of that process are being communicated separately to them.

We recognize your privacy is important. As such, the university will only track logins to the assessment; specific answers to questions will remain anonymous. Logins are tracked for the purpose of determining if employees have completed the survey as required. Responses are tracked at the aggregate level for the purpose of assessing the health and well-being of our overall university community. The university will not link individual responses with personally identifiable information.
