Forms

• Program Release Form (pdf) - All children attending any of the tracks will need to have a signed program release form submitted before the start of the event.  You can send the completed and signed form to toctwd@andrew.cmu.edu.  We encourage everyone to fill out and submit this prior to the day of the event.
• School Information Form (pdf) - This form can be submitted to your childrens school as proof that they attended the event here at CMU.

We are always looking for volunteers to either help the day of the event if you are interested please email toctwd@andrew.cmu.edu . Thank you for your consideration!
