Request for Dependent Information Email
Starting on December 8, 2017, HR Benefits will be emailing faculty and staff members who have obtained CMU-sponsored medical coverage for a dependent and have not yet provided that dependent's Social Security Number (SSN) to the university. Please call HR Benefits using the phone number provided in the email. Please do not reply to the email or email the dependent's SSN.
The Affordable Care Act requires insurers and employers to report to the IRS the SSNs of all employees and dependents who have obtained minimum essential health coverage. The IRS uses this information to determine whether an individual is subject to the "individual mandate" penalty tax for not having health insurance. The IRS may impose a penalty if this information is not received.
Please note that this only applies to faculty and staff members who receive an email from HR Benefits regarding this issue.
