TRANSFER CELL PHONE LIABILITY
FROM UNIVERSITY TO PERSONAL

AT&T - SPRINT - T-MOBILE - VERIZON WIRELESS

To start the process of transferring the liability from the University to your individual account, please have your account number available in addition to the mobile number and have other information that would be used in a credit application.

If your wireless service provider is not listed above, each individual or department customer is responsible to contact their provider's Customer Service Center for instructions on completing the liability transfer process from Carnegie Mellon University to the individual.

Insurance Requirements for Local Transportation Service Suppliers
Please plan as far in advance of your event as possible so that your Department and the University can obtain the best value in service and protection.

Preferred Suppliers provide more than adequate insurance coverage to protect the University in the event of an accident or other claim. But, if a Department is planning to use the services of a non-Preferred supplier, it is the Buyer’s responsibility to obtain a certificate of insurance from the supplier, and to send it to Brian Cappo, Director, Risk Management, for approval. If the supplier meets the University’s requirements, the Buyer will need to request a new certificate which names Carnegie Mellon as an additional insured for the specific event. -5/12/2008
