Card Issuance

All ID cards are issued by the Carnegie Mellon Card Office (or their designated agent). The first Carnegie Mellon ID+ Card (for faculty, staff, students, emeriti and retirees) is issued at no charge. Affiliate Cards are issued to the card sponsor at the standard fee of $15.

Valid photo identification, such as a driver’s license or passport, must be presented to obtain an ID card. The Carnegie Mellon Card Office staff will verify card eligibility based on enrollment and/or employment status in University systems at the time of card issuance.

New Students for the Fall Term

Prior to the upcoming school year we request that students submit a photo that we will use to create the students ID+ card prior to the students arrival on campus.  Photos should be a standard headshot with the student by themselves in the main area of the photo.  Students are not allowed to have caps or hats on their ID+ card photo.  There is no specific size requirement for the photo as we will crop the photo appropriately for use on the ID+ Card.  Digital photos can be submitted through our website by visiting our ID Photo Submission Application

Undergraduate Students who are entering the university in the fall term and who sent in a photo for their ID+ card in advance of their arrival on campus will receive their ID+ card at the time of their orientation.  Students living in campus housing will receive their cards as part of their housing check in process.  Commuter students will receive their card at a separate orientation session.  Students who did not send in a photo in advance of their arrival on campus will be given a notice in their welcome packet that indicates that they must visit the Card Office during normal business hours in order to have a photo taken and their ID+ Card produced.

New Graduate Sudents should check with their department to determine if the department will be distributing ID+ cards at an orientation session or if they should come to the Card Office to request their card.  If the department indicates that the student should acquire their card from the Card Office then the student can come to the office anytime after August 1st during normal business hours and complete the Identification Card Request Form and the card office will issue the card at that time.

Student (other than new fall term students), Faculty, or Staff ID+ Card Issuance

An Identification Card Request Form (available in the Card Office) must be completed and signed at the time of issuance.

Affiliate Card Issuance

The Affiliate Identification Card Request Form must be completed with the appropriate signatures for all affiliate cards. 

  • Student Spouses and Domestic Partners – Student spouses must present either a marriage license or an I20 with both the student and spouse’s names on it at the time of issuance.  Student domestic partners must submit a completed Registration Statement of Domestic Partnership of Carnegie Mellon University Students Form, including notary authorization, at the time of issuance.  One of these forms must be presented along with a completed Affiliate Identification Card Request Form signed by both the person that is requesting the card and their sponsor.
  • Faculty and Staff Spouses, Domestic Partners, and Family Members – Benefits-eligible faculty and staff affiliates must present a completed Request for ID Cards for Family Members/Registered Domestic Partners Form, including Human Resources approval, at the time of issuance along with a completed Affiliate Identification Card Request Form.
  • Visiting Researchers and Scholars – Visiting researchers and scholars must present the completed Affiliate Identification Card Request Form with both their signature and a signature from their department at the time of issuance.  If the department will be entering the cardholder into the Human Resource Employee Management (HREM) the department must indicate this on the request form and the HREM record will have to be completely processed prior to the card being issued.
  • Non-degree Students – Non-degree students must present the completed Affiliate Identification Card Request Form with both their signature and their instructor’s signature at the time of issuance.
  • Vendors and Contractors – Vendors and contractors must present the completed Affiliate Identification Card Request Form with both their signature as well as a signature and appropriate charge string from the sponsoring department.

If an individual’s status changes a new ID card may be requested at no charge, provided the previous card is presented.  Otherwise, the standard replacement fee applies.
