Affiliates/Other
Affiliate Cards are available as a courtesy to certain family members of benefits-eligible faculty and staff, student spouses, and registered domestic partners. Non-degree students, university vendors and contractors, and campus visitors are also eligible for Affiliate Cards. Affiliate cardholders cannot obtain Affiliate Cards for anyone. All Affiliate Cards have a maximum expiration date of one year after the card issue date, and they must be renewed annually in order to confirm the continuing eligibility of the cardholder and their sponsor.

Faculty & Staff Family Members
Benefits-eligible faculty and staff can obtain Affiliate Cards for spouses or domestic partners who meet certain eligibility criteria and for their dependent children (aged 12 to 21).
Spouse or Child
- Complete an ID Card Request - Affiliates form.
- Complete a Request for ID Cards for Family Members/Registered Domestic Partners form.
- Have it signed by your sponsor on the Authorizing Personnel Signature line.
- Return it to The HUB along with a $15 Affiliate Card fee.
- Present government issued photo identification (driver's license or passport).
- Affiliate will have his/her picture taken at The HUB and will receive the Affiliate Card at that time.
Domestic Partner
- Register with Carnegie Mellon Human Resources by completing a Registration Statement of Domestic Partnership.
- Complete an ID Card Request - Affiliates form.
- Have it approved and signed by Human Resources.
- Return it to The HUB along with a $15 Affiliate Card fee.
- Present government issued photo identification (driver's license or passport).
- Affiliate will have his/her picture taken at The HUB and will receive the Affiliate Card at that time.
Student Spouses & Domestic Partners
Degree-seeking students can obtain Affiliate Cards for spouses, children (ages 12-21) or domestic partners who meet certain eligibility criteria.
Spouse or Child
- Complete an ID Card Request - Affiliates form.
- Students must present proof of marriage license or an I-20 for dependents.
- Have it signed by your sponsor on the Authorizing Personnel Signature line.
- Return it to The HUB along with a $15 Affiliate Card fee.
- Present government issued photo identification (driver's license or passport).
- Affiliate will have his/her picture taken at The HUB and will receive the Affiliate Card at that time.
Domestic Partner
- The prospective cardholder and domestic partner must complete the Registration Statement of Domestic Partnership form.
- Complete an ID Card Request - Affiliates form.
- Have it approved and signed by Human Resources.
- Return it to ID Card Services along with a $15 Affiliate Card fee.
- Present government issued photo identification (driver's license or passport).
- Affiliate will have his/her photo taken at The HUB and will receive the Affiliate Card.
Visiting Researchers & Scholars
Departments can obtain Affiliate Cards for visiting researchers and scholars who will be on campus for more than three weeks. It is requested but not required that visiting researchers and scholars requiring an ID Card have a sponsored appointment in the Human Resources Employee Management (HREM) system in order to facilitate the creation of an ID Card account. There will be an additional $10 fee for requests that are not entered into the HREM system. Researchers and scholars who will be entered into HREM must wait until their record is completely processed before requesting a card from The HUB.
Prospective cardholders may only be sponsored by a university department or other organizational unit (not by an individual). Their spouses, children or other family members are NOT eligible for Affiliate Cards.
To get a card:
- The department must complete an ID Card Request - Affiliates form, on behalf of the visiting researcher or scholar. The form must contain a GL Oracle account number to satisfy the $15 Affiliate Card fee ($25 if the individual is not going to be entered into HREM/HRIS).
- The department must have the visiting researcher or scholar visit The HUB.
- Present government issued photo identification (driver's license or passport).
- The visiting scholar or researcher will have his/her photo taken in The HUB and will receive the Affiliate Card at that time.
Non-Degree Students
Program Participants & Conference Attendees
Departments can obtain Affiliate Cards for special program participants and conference attendees. These cards are either photo or non-photo ID Cards, which are issued for a limited time period. Photo ID Cards are required for programs of three weeks or longer in duration. Departments may choose either photo or non-photo ID Cards for programs/conferences that are shorter than three weeks in duration. Departments are responsible for distributing these cards to participants and attendees; and for collecting and returning them to ID Card Services when the programs are completed.
Prospective cardholders must be special program participants or conference attendees of a university department sponsored program or conference. Participant/attendee's spouse, children or other family members are NOT eligible for Affiliate Cards.
To get a card:
- The department must complete an ID Card Request - Affiliates form, on behalf of the program/conference participants/attendees and have it signed by the department head. The form must contain a GL or GM Oracle account number to satisfy the Affiliate Card fee ($15 Per cards).
- The department must attach a list of the participants' or attendees' first and last names.
- The department must return the form to The HUB at least two weeks before the group's arrival.
- Non-photo ID Cards will be delivered to the department for distribution.
- Photo ID Cards can be given to the department for distribution. The department will assume responsibility of the IDs upon pickup from the HUB.
- Must present government issued photo identification (driver's license or passport) in order to have their photo taken at that time and receive their Affiliate Card.

Emeriti/Retiree
Authorized Designate Renewal
If a cardholder is unable to visit The HUB to renew their ID Card, the cardholder may authorize a designate to renew their ID Card for them. The cardholder must complete the ID Card Request - Affiliates form, as well as the Authorized Designate Renewal form. The designate must bring both completed forms to The HUB, along with the cardholder's current ID Card. The designate must be the individual named on the designate form and must present their valid ID Card. When we have completed printing the replacement ID Card for the cardholder, the designate will be required to complete and sign the ID Card acceptance portion of the designate form.
A department is allowed to appoint a designate to renew multiple cards for a single department as long as the designate is willing to accept responsibility for the ID Cards until they are delivered to the appropriate cardholder. An ID Card Request form and Authorized Designate Renewal form must be completed for every ID Card that the designate will be renewing.
Designates must deliver the ID Cards to the cardholders in person. Under no circumstance is the designate allowed to leave the ID Card in a campus mailbox or to have the card delivered by an external mail/package delivery mechanism such as the USPS, UPS or FedEx. Once the card is signed for, the card is now the responsibility of the designate.
