Affiliate Cards
How do I get an Affiliate Card?
Spouse or child
The prospective cardholder must be the spouse or child of any employee, or spouse of a degree-seeking student. Children must be older than 12-years-old and younger than 21-years-old.
To get a card:
- Complete an Affiliate Identification Card Request form.
- Complete a Request for ID Cards for Family Members/Registered Domestic Partners form (faculty and staff only). Students must present proof of marriage license or and I-20 for dependents.
- Have it signed by your sponsor.
- Return it to ID Card Services in The HUB along with a $15 Affiliate Card fee.
- Present government issued photo identification (driver's license or passport).
- You will have your picture taken at The HUB and will receive your Affiliate Card at that time.
Domestic partner (faculty/staff)
The prospective cardholder must be the registered domestic partner of an employee. Both partners must register with Carnegie Mellon Human Resources by attesting, at a minimum*, to the following statements:
- Both parties are 18 years of age or older and are mentally competent at the time of registration.
- Neither party is legally married to anyone else.
- The parties are not related by blood to a degree that would bar marriage in the Commonwealth of Pennsylvania.
- The relationship has been entered into voluntarily, willingly and without reservation.
- The relationship is the functional equivalent of marriage including: living together as a couple, mutual support, mutual caring and commitment, mutual fidelity, and mutual responsibility for each other's welfare.
- The partners have been living together as a couple for at least three months prior to registration with the university. The relationship is intended to continue indefinitely but is terminable at the will of either partner.
*Attesting to the above statements entitles domestic partners to an Affiliate Card. Domestic partners may be entitled to additional benefits, such as health insurance coverage, if they can attest to certain additional conditions.
To get a card:
- Register with Carnegie Mellon Human Resources by completing a Registration Statement of Domestic Partnership.
- Complete an Affiliate Identification Card Request form.
- Have it approved and signed by Human Resources.
- Return it to ID Card Services in The HUB along with a $15 Affiliate Card fee.
- Present government issued photo identification (driver's license or passport).
- You will have your picture taken at The HUB and will receive your Affiliate Card at that time.
Domestic partner (student)
To get a card:
- The prospective cardholder and domestic partner must complete the Registration Statement of Domestic Partnership form.
- Complete an Affiliate Identification Card Request form.
- Have it approved and signed by Human Resources.
- Return it to ID Card Services along with a $15 Affiliate Card fee.
- Present government issued photo identification (driver's license or passport).
- You will have your photo taken at The HUB and will receive your Affiliate Card.
Visiting researchers & scholars
Prospective cardholders must be visiting the Carnegie Mellon campus for more than one week in the capacity of a visiting scholar or researcher. Prospective cardholders may only be sponsored by a university department or other organizational unit (not by an individual). Their spouses, children or other family members are NOT eligible for Affiliate Cards.
To get a card:
- The department must complete an Affiliate Identification Card Request form, on behalf of the visiting researcher or scholar. The form must contain a GL Oracle account number to satisfy the $15 Affiliate Card fee ($25 if the individual is not going to be entered into HREM/HRIS).
- The department must have the visiting researcher or scholar visit ID Card Services in The HUB.
- Present government issued photo identification (driver's license or passport).
- The visiting scholar or researcher will have his/her photo taken in The HUB and will receive his/her Affiliate Card at that time.
Program participants & conference attendees
Departments can obtain Affiliate Cards for special program participants and conference attendees. These cards are either photo or non-photo ID Cards, which are issued for a limited time period. Photo ID Cards are required for programs of three weeks or longer in duration. Departments may choose either photo or non-photo ID Cards for programs/conferences that are shorter than three weeks in duration. Departments are responsible for distributing these cards to participants and attendees; and for collecting and returning them to ID Card Services when the programs are completed.
Prospective cardholders must be special program participants or conference attendees of a university department sponsored program or conference. Participant/attendee's spouse, children or other family members are NOT eligible for Affiliate Cards.
To get a card:
- The department must complete an Affiliate Identification Card Request form, on behalf of the program/conference participants/attendees and have it signed by the department head. The form must contain a GL Oracle account number to satisfy the Affiliate Card fee ($15 for all cards).
- The department must attach a list of the participants' or attendees' first and last names.
- The department must return the form to ID Card Services at least two weeks before the group's arrival.
- Non-photo ID Cards will be delivered to the department for distribution.
- Photo ID Cards will require the participants/attendees to come to The HUB.
- Must present government issued photo identification (driver's license or passport) in order to have their photo taken at that time and receive their Affiliate Card.
What do Affiliate cardholders have access to?
Affiliate cardholders have access to athletic facilities and equipment; campus academic buildings; tickets for athletic events, concerts, films and other activities (as long as they are not limited to students only); and university libraries borrowing privileges. Special program participants and conference attendees' university libraries borrowing privileges must be arranged in advance by the department sponsoring the program. Access to athletic facilities and equipment will be made available to conference participants for a fee determined by University Athletics.
When do Affiliate Cards expire?
Affiliate Cards expire one year from the date of issue. Cardholders can renew expired cards each year they are eligible by completing a new Affiliate Identification Card Request form and exchanging the expired card at no additional cost.
Special program participants/conference attendees and visiting researchers/scholars affiliate cards are valid only for the duration of the special program or conference. Departments must collect cards from the cardholders at the expiration of the visit and return them to ID Card Services for destruction.
