Returning from a Leave of Absence

When you plan to return to Carnegie Mellon after a leave of absence, you must complete a Return from Leave form.

Several signatures are required on this form prior to processing by Enrollment Services. Sent the completed form to your department for appropriate signatures, request that they forward the form to your dean and to the Dean of Student Affairs, if required. The final signing office will forward the form to Enrollment services for processing.

If your have an outstanding student account balance, your form will not be completely processed until you contact the Assistant Directors of Enrollment Services at 412-268-8186 or via email.

Once your application is processed, you will recieve additional information from Enrollment Services.

Your Andrew UserID will be active upon completed processing of your Return from Leave form.

Transfer Credits

If you have transfer credits earned during your leave, send an official transcript to:
