University Registrar's Office Change of Name

If you are a current student or a former student and wish to change the official name on your academic record, you must complete this form. Please note:

  • This form may be used to change the name on a transcript, diploma, Ph.D. thesis or other university record, such as a former student record.
  • Carnegie Mellon is unable to change the name that appears on records that were published by third-party entities.
  • Reasons to complete this form may include the following: adoption, marriage, divorce, naturalization, passport updates, corrections, and/or legal name changes.
  • Legal documentation is required to make this change.

Based on the level of sensitivity, value, and criticality to the University, as required by the University's Information Security Policy, the data associated with this request, including uploaded files, is considered restricted data, and therefore, the highest level of security controls are applied for the protection of data.
