The first step toward arranging accommodations is to complete and return the Student Voluntary Disclosure of a Disability Form. Once the completed form is returned, Disability Resources will contact the student to discuss disability issues and determine the appropriate documentation. It is then the responsibility of the student to forward a copy of the documentation to Disability Resources. Upon receipt of the documentation, Disability Resources will work with the student to develop an Individualized Accommodation Plan and an Accommodation Memorandum. The Individualized Accommodation Plan details specific physical and learning issues; a copy is given to the student to share with the campus community as the individual sees fit. The accommodation memorandum lists specific accommodations required by the student. It is the student's responsibility to share this document with each of his/her instructors.

The Intake/Accommodations Request Form (.pdf) is used to add contact information to the Disability Resources Database, which allows us to update individuals regarding special events, disability information and accommodation issues and concerns.
