Davis Vision and Vision Benefits of America (VBA) administer our vision programs. Our vision options are designed to give you flexibility in coverage and providers.

Vision Benefits Information
Davis Vision
- Obtaining Vision Care:
During an appointment with a participating provider, show the provider your Davis Vision member ID card. The provider will submit the claim directly. When using a non-participating provider, pay for the service in full, and then submit a Reimbursement Claim [pdf] at the out-of-network level. - Davis Vision Plan Information: Search for providers, review benefits, view frame collection, learn about laser vision correction or Lens 1-2-3, or take a satisfaction survey. Be sure to log in with the appropriate control code.
Vision Benefits of America
- Obtaining Vision Care:
Most participating providers can submit electronic claims. When using providers without electronic claims filing, you must request a benefit form before seeking services. If you do not get a benefit form prior to your appointment, the visit will be considered out-of-network. Request a benefit form online or by calling 800-432-4966. Bring the benefit form to your appointment. For out-of-network services, pay in full and then submit a benefit form and receipts for out-of-network reimbursement. - VBA Plan Information: Review benefits, learn about laser vision correction or request a benefits form.

HMO Medical Plan: Vision Benefits
The HealthAmerica HMO provides for one routine eye exam with refraction every 12 months for participants under age 18, through the EyeMed Vision Care network (866-723-0514 or http://www.eyemedvisioncare.com). Check your plan document or contact the carrier for details about the vision benefits included in the HMO plan.
