Davis Vision and Vision Benefits of America (VBA) administer our vision programs. Your vision selection is a two-year election. Your current vision election will be in effect through December 31, 2009. Our vision options are designed to give you flexibility in coverage and providers.

  • Obtaining Vision Care:
    During an appointment with a participating provider, show the provider your Davis Vision member ID card. The provider will submit the claim directly. When using a non-participating provider, pay for the service in full, and then submit a Reimbursement Claim (.pdf) at the out-of-network level.

Additional Vision Services: HMO Benefits

The HMO programs include limited vision benefits. HealthAmerica HMO utilizes the EyeMed Vision Care network (866-723-0514 or http://www.eyemedvisioncare.com. Check your plan documents or contact the carrier for details about the vision benefits included in the HMO plans.
