(For Full-Time Benefit-Eligible Faculty and Staff)

United Concordia Companies, Inc. (UCCI) administers our dental programs. Your dental plan selection is a two year election. Your current election will be in effect through December 31, 2009.

Our dental options include two dental PPOs (ConcordiaFLEX plans) and a dental HMO (a ConcordiaPLUS plan). To access services, present your United Concordia ID Card to your dentist. There is no special claim form; your dentist should use a standard American Dental Association claim form.

* - You can ask your dentist directly if s/he participates in the network, or contact UCCI at 1-800-423-7461 (for a PPO plan) or 1-866-357-3304 (for the DHMO). If your dentist does not participate in the UCCI plan, you can nominate him/her at the UCCI website.
