Submitting 2009 Claims to EBDS
Claims incurred in 2009 that are submitted by December 31, 2009 should be sent to EBDS using the process described below. Claims for expenses incurred in 2009 that are submitted between January 1 - June 30, 2010 should be submitted to Ceridian Benefit Services.
Automatic Claims Filing for Caremark and Highmark Expenses
Please note that unless you turn off automatic claims filing from the EBDS website, the following claims will be automatically filed (and you will be sent payment):
- Caremark prescription expenses
- Highmark medical expenses for Carnegie Mellon Highmark participants.
Submitting Claims for Eligible Expenses That Are Not Automatically Submitted:
- Save the Explanation of Benefits (from your insurance carrier) or the receipt (from the provider/store). Receipts that are not on an 8 1/2 x 11 sheet should be copied onto this size paper.
- Complete the online claims submission from the EBDS website -OR-
complete the appropriate sections of the EBDS FSA Claim Form (.pdf).
You may submit multiple expenses on one claim form.
- FAX copies of your supporting documentation (and your claim form, if not submitted online) to 866-309-7463 or 866-309-8906 -OR- submit them by mail to: Account Service Center, P.O. Box 22130, Pittsburgh, PA 15222-0158.
- EBDS will process the claim and mail you a check (minimum check is $25). You can establish Direct Deposit by completing the Direct Deposit Authorization Form in hard copy or at the EBDS website.
- Claims submitted by December 31, 2009 should be submitted to EBDS. Claims for expenses incurred in 2009 that are sent from January 1 - June 30, 2010 should be submitted to Ceridian.
- Any money remaining in your account for which claims are not filed by June 30 of the following year will be forfeited, as per IRS regulations.
