Vision plans
Carnegie Mellon has contracted with Highmark to provide excellent and affordable student vision insurance. Students can enroll in the vision insurance plan during the Fall or Spring open enrollment periods only.
The plan details match those of the 2011-2012 year, but... premiums have gone down for 2012-2013.
Plan name: Highmark's Fashion Advantage Vision Program
Coverage period: August 1 - July 31 for Fall enrollees, Jan 1 - July 31 for Spring enrollees.
Students must appear in the Highmark Fashion Advantage system before generating a claim, or the claim will not be paid. Enroll early so you have your card before you wish to make use of the vision insurance. Students receive new vision insurance cards each year. Old cards cease to work when the new plans start each August 1.

- Two-person (Individual and spouse, or individual and one child): $144.20
- Family: $213.80

To locate a provider, go to www.highmarkbcbs.com and select "Find a network vision provider." Click "OK" to be redirected to the Davis Vision, Inc. Web site. Enter your zip code and mile radius then click on "Search" to see the most current listing of providers that will accept your vision plan.
