Carnegie Mellon's medical, vision and dental student insurance plan(s) run August 1 through the following July 31 each year.
Plan Level Definitions
Individual: enroll yourself
Two-person: enroll yourself and one other family member (a spouse or domestic partner or your dependent child)
Family: enroll yourself and at least two other family members
NOTE: Dental insurance has no two-person level, so Dental Family level enrolls you and at least one other family member.

Finding providers in the networks
All of the student insurance plans are network-based. Physicians, hospitals, and other health care providers in the networks have contracted to provide specific medical care at negotiated prices. It is important to ALWAYS make sure that the providers you choose are in your plan's network.
You need to make sure all non-emergency visits or tests of any kind are provided by doctors and facilities in the network to make sure you get the highest level of benefit.
Medical: Network is Highmark PPO Blue
For providers within the United States: visit Highmark's online national provider search engine to locate network doctors and facilities
To locate international network doctors and facilities: visit Highmark's online worldwide search engine.
Overseas medical assistance: Visit BlueCard WorldWide (or view this BlueCard Worldwide brochure) to obtain medical assistance, inpatient, outpatient and professional services. Mondial (World Access) is used by providers and the BCBS plans to determine benefits, process claims, translating bills and currency and payment scheduling/payment remittances, etc.. This organization also handles medical evacuation for our student plan.
Vision: Network is Davis
The dental coverage is for providers within the United States only. It does not provide coverage overseas. To find providers, go to www.unitedconcordia.com, and select 'Find a dentist' in the left most corner of the page. Enter 'Concordia Advantage'as your network and then specify your search criteria.

The dental coverage is for providers within the United States only. It does not provide coverage overseas. To locate a provider, go to www.highmarkbcbs.com and select "Find a network vision provider." Click "OK" to be redirected to the Davis Vision, Inc. Web site. Enter your zip code and mile radius then click on "Search" to see the most current listing of providers that will accept your vision plan.

Highmark Cost Estimator
To estimate your costs:
- Create your Highmark website account. Visit www.highmarkbcbs.com, and select the MEMBERS tab. In the red LOG IN box, click on REGISTER NOW. Follow the steps to create your account. You need your Highmark medical id number in order to set up this account.
- Login to your Highmark website account, and select the "your spending" tab. On the left hand side you'll see "typical medical cost." Follow the directions to enter appropriate info regarding the fee you wish to estimate
