Helpful Information for New Students
Health History & Immunization Requirements
All incoming students must complete Health History and Immunization Forms (see Accessing the Forms below). Carnegie Mellon University requires that all incoming students have the following immunizations before arrival on campus:
Full time students:
- Two doses of measles, mumps and rubella vaccine (MMR)
Additional requirements if living in university housing:
- Hepatitis B vaccination series
- Meningitis vaccination
Additional requirements for international students:
- Tuberculin skin test (PPD) within six months of enrollment.
If you are unable to obtain these vaccinations prior to arrival on campus, you can receive them from University Health Services prior to the first day of classes. Failure to comply will result in administrative suspension and removal from university housing.
Accessing Health History & Immunization Forms
Our immunization forms are available in HealthConnect, but you need to have your andrew id and password for access. Also, your birthdate in the Student Information System (SIO) must be correct. If you are having trouble connecting to HealthConnect, check your birthdate in SIO. If it is incorrect, correct it at the HUB. The following day this modified info should be in our HealthConnect system. If you still have problems, then contact us for help.
When you have your andrew id, visit
. Select Forms, and you'll see the list of forms we need you to complete. In addition to the health history and immunization forms, you'll also need to acknowledge online receipt of our Notice of Privacy and Treatment Agreement documents.
Special Dining & Housing Requests
- If you have special nutritional needs and require a dining plan accomodation, submit a Special Dining Needs Form [.pdf]
- If you have a health condition that requires special housing accomodations, submit a Special Housing Needs Form [.pdf]
