All new students are required to complete Health History and Immunization forms. These allow University Health Services to monitor the immunization status of our students to ensure that all are protected against vaccine-preventable diseases.
Accessing the forms - Immunization forms are available at HealthConnect. In order to access HealthConnect, students must have an andrew id and password, and a correct birthdate in the Student Information System (SIO). If you cannot access HealthConnect, check the birthdate in SIO. If it is incorrect, correct it at the HUB and it should be modified in the HealthConnect system the following day. If you still have problems, then contact us for help.
When you have your andrew id, visit
. Select Forms to see the list of required forms. In addition to the health history and immunization forms, students also need to acknowledge online receipt of our Notice of Privacy and Treatment Agreement documents.
Immunization requirements - Carnegie Mellon University requires these immunizations before arrival on campus:
All full time students:- Two doses of measles, mumps and rubella vaccine (MMR)
- Hepatitis B vaccination series
- Meningitis vaccination
- Tuberculin skin test (PPD) within six months of enrollment
If you are unable to obtain these vaccinations before arrival on campus, you can receive them at University Health Services prior to the first day of classes.
Failure to comply will result in administrative suspension and removal from university housing.

Students with special nutritional needs that require a dining plan accomodation must submit a Special Dining Needs Form [.pdf].
Students with special health needs that require a housing accomodation must submit a Special Housing Needs Form [.pdf].

AlcoholEdu
AlcoholEdu is an online alcohol education program that all first-year students are expected to complete in the fall term of their first year. We recognize the important role parents play in helping their children make good decisions about alcohol use. We encourage you to have conversations with your student about alcohol before school starts, and invite you to preview the AlcoholEdu program.
Parents can preview the AlcoholEdu program by entering user ID: 11C211440PARENT at college.alcoholedu.com.
Tips for Packing a First Aid Kit
- Antiseptic skin cleaner (betadine)
- Acetaminophen for pain relief and fever
- Decongestant like sudafed
- Antibiotic cream or ointment
- Antihistamine for itching, allergies (e.g. Benadryl)
- Antacid tablets or liquid for upset stomach
- Band-Aids in assorted sizes
- Sterile gauze pads, 4 X 4 and a roll of 3-inch gauze
- Adhesive tape
- Cotton swabs
- Thermometer
- Scissors
- Tweezers
- Ice pack
- Safety pins
We recommend that contact lens wearers also bring a pair of glasses to school with them.
