In order to obtain any of the forms listed below, individuals must contact the Tax Manager directly:

• Sales Tax Exemption Certificates
• Form W-9
• Form 990’s (June 30, 2007)
• Form 990-T’s (June 30, 2007)
• IRS Determination Letter

The following forms are available to CMU faculty/staff. They are hosted at the Finance Division site to provide a central location and help when trying to search for a specific form:

• Sale of Property Form
• Gift Tax Forms
• Nonqualified Scholarship Forms
