Financial Aid Eligibility & Missing Documentation

Financial aid eligibility cannot be finalized if required documentation is missing. If you've been identified as one of these students, you will receive a Financial Aid Alert email from The HUB notifying you that documents are missing from your financial aid application. Instructions for submitting missing documents may be found in the chart below. 

IMPORTANT: If you have not yet submitted your INITIAL packet of documents to CSS/IDOC, please follow the instructions in the gray box titled 'Submit Initial CSS/IDOC Packet' at the bottom of this page.

FAFSA Complete online at https://fafsa.ed.gov
Federal Master Promissory Note (MPN) and/or Entrance Counseling

Complete online at https://studentloans.gov. Instructions can be found here (pdf).

*If you do not wish to borrow Federal Direct Loans, please complete a Loan Adjustment form and submit it to The HUB. 

Perkins Loan Master Promissory Note (MPN) and/or Entrance Counseling

To sign your Perkins Loan MPN and complete Entrance Counseling, please go to https://www.ecsi.net/promZ9. Instructions can be found here (pdf).

In order to sign your MPN electronically, you will need to validate your identity by providing your Social Security Number, last name, first name, and date of birth. If your identity cannot be validated, you will be unable to electronically sign your MPN and, instead, you will be instructed to print the MPN, complete it in full, and deliver it to The HUB (located in the lower level of Warner Hall).

Note: Complete reference information for yourself, next of kin (relative), and two additional references must be provided when completing your MPN. Failure to complete all fields will delay disbursement of your loan to your account.

Attestations Log in to Student Information Online (SIO)hover over 'Finances', and select 'Financial Aid Permissions.' Log into the Financial Aid Portal by clicking on the blue 'complete attestations here' link.
CSS PROFILE

If applying for institutional financial aid programs, complete the CSS PROFILE online at student.collegeboard.org/profile. If you do not have a College Board online account, you will need to create one in order to register for the PROFILE. Carnegie Mellon receives your PROFILE data electronically from CSS. Carnegie Mellon's CSS College Code is 2074.

Noncustodial Parent PROFILE Complete the CSS Noncustodial PROFILE (NCP) online at ncprofile.collegeboard.comThe student’s CSS ID and Noncustodial PROFILE password that were assigned to you as part of the CSS PROFILE process will be needed to sign in to the NCP. Carnegie Mellon receives your Noncustodial PROFILE electronically from CSS.
Parent/Student Tax Returns and/or W-2s

Submit these to IDOCS in one of the following ways:

  1. Upload electronic versions at https://idoc.collegeboard.org/idoc/ (preferred method); or
  2. U.S. Postal Service to: College Board Processing Center, P.O. Box 8570, Portsmouth, NH 03802

Please note that handwritten signatures and dates are required on page 2 of each Form 1040. You may log into your College Board CSS/PROFILE account and select “Check Status” at the top of the page to verify that CSS/IDOC received your documents. If your submitted documents are not reflected under your account, please contact CSS/IDOC. 

Parent/Student Tax Return Transcript This is required if selected for federal verification. We recommend requesting a transcript online (view detailed instructions). 
Non-Tax-Filer Statement

Complete a Non-Tax-Filer form and submit to The HUB. This form may be found on the Forms & Guides webpage under General Financial Aid.

Parent PLUS Application Apply online at https://studentloans.gov by selecting 'Parent Borrowers' and 'Apply for a PLUS Loan'. 
Parent PLUS Master Promissory Note (MPN) and/or Entrance Counseling Complete MPN online at https://studentloans.gov by selecting 'Parent Borrowers' and either 'Complete PLUS Loan Master Promissory Note (MPN)' or 'Complete PLUS Counseling'.
Unsub or Parent PLUS Loan Only Print, complete and submit the application to The HUB. 
Parent/Student Form 1099 Complete and submit a copy to The HUB. 
Schedules A, B, C, D, E, K-1 Complete and submit a copy to The HUB. 
Selective Service Registration Register online at www.sss.gov and submit the confirmation page to The HUB.
U.S. Citizenship Documentation

Submit an original copy of U.S. Citizenship Documentation (i.e., U.S. Passport, Certification of Naturalization) in person to the HUB. If you are unable to appear in person, complete the 2017-18 U.S. Citizen/Eligible Non-Citizen Authentication of Documentation form and submit it to the HUB. This form may be found on the Forms & Guides webpage under Verification.

DHS Verification Document (G845)

The United States Citizenship and Immigration Services (USCIS) of the Department of Homeland Security did not confirm that you are a noncitizen (Item 14) in an immigration status associated with the requirements of eligibility for the financial assistance for which you have applied. You must submit proof to CMU that you are in the required noncitizen immigration status. Please contact your HUB liaison for further instructions. 

Form 399 Conflicting Information

The amount reported for the student's income and/or parents' or spouse's income, and the amount reported for taxes paid has changed from the amount reported on last year's FAFSA. Corrections need to be made to the Student Aid Report (SAR) (information received after FAFSA completion). Please contact your HUB liaison for further instructions. 

NSLDS Match

Comments received on your Student Aid Report (SAR) (information received after FAFSA completion) need to be reviewed and necessary action should be taken in order to resolve any discrepancies. Please contact your HUB liaison for further instructions. 

Identity and Statement of Educational Purpose

Submit a valid government issued photo ID and the Statement of Educational Purpose in person to the HUB. If you are unable to appear in person, complete the 2017-18 Identify and Statement of Educational Purpose form and submit it to the HUB. This form may be found on the Forms & Guides webpage under Verification.

Submit Initial CSS/IDOC Packet

If you have not already submitted your initial packet of tax documents to CSS/IDOC for the 2017-18 academic year, then you should submit the following along with the CSS/IDOC cover sheet provided to you via email after your CSS PROFILE completion:

  • Parent 2015 tax return
  • Parent 2015 W-2 form(s)
  • Student 2015 tax return (if required to file)
  • Student 2015 W-2 form(s) (if applicable)
  • Schedule C and K-1 from Forms 1065 and/or 2555 (if applicable)

Documents can be submitted to CSS/IDOC in one of the following ways:

Eligibility Notification (Award Letter)

You will receive an email notification to your CMU Andrew email account once your award letter is available to view through Student Information Online (SIO).
