Invention Disclosure Forms

If you are interested in transferring materials from your lab to another University and/or Company, please complete the form and email back to gorana@cmu.edu.

Once our office receives your form, we will post it to Flintbox, a web-based application that allows us to market your materials and collaborate with others academic institutions and/or industry.

Before you download, please verify if the institution you are working with is a participant of the UBMTA by clicking here.

Please be sure to send a copy for our office to have on file.
