Step 1: Create the HTML Form
The first step is to create an HTML form. You can use a web page development package such as Dreamweaver to create the content and layout of your form. If you choose to do so, it is recommended that you edit the HTML of the form to give the fields logical names. Web authoring tools such as Dreamweaver tend to assign fields a default name. Changing these to a name that makes sense to you will make future editing much easier.
When naming a field, remember that field names are case sensitive and must be all one word (no spaces). If you need help writing forms in HTML you may want to refer to the WC3 Form tutorial.
