New Membership Form
Alumni looking to join the Carnegie Mellon Admission Council for the first time should complete this form. Previous members that have discontinued their membership should also complete this form to rejoin CMAC. Please provide contact information that will be current from September 1st through July 31st.
Renewal/Correction Form
Existing members of CMAC looking to update their contact information, preferred method of contact or participation levels should complete this form. This form should also be used to cancel an existing CMAC membership. Please note that your information will remain in the printed directory until next year, but will be promptly removed from the Web site.
