Forms for Freshmen and New Athletes

ImPACT Test - please visit this link to take the ImPACT test prior to arrival on campus

By NCAA, and our University standards, all new athletes are required to get a
physical before athletic participation.

Please print and fill out forms A, A1, B, C and the health history questionairre.
Take Carnegie Mellon's Athletic physical form with you to a medical doctor for them to fill out and sign.
Mail back completed forms by July 15th to:
